1. How did you hear about Great Pyrenees?
2. Why do you want a Great Pyrenees?
Are you familiar with the common "objectionable habits" of Great Pyrs?
(barking, digging, shedding vast amounts of hair, tendency to guard people or
animals especially the young and babies, tendency to wander or roam, drooling,
independent nature?
3. Have you recently applied to other rescue organizations or animal
shelters?
If so, which ones and what is the status of the application(s)?
4. List any other pets you currently own or have living in your home.
5. Are your dogs on heartworm preventative?
If so, what type?
6. Are you familiar with heartworm disease and the detrimental effects it
can have on your dog?
7. List any pets you have had in the past 5 years and where they are now.
8. How many children in your household?
Ages?
9. Do you have any other children or grandchildren that will be visiting
on a regular basis? Ages?
10. How many adults in your household?
11. Do all adults agree on the adoption of a Great Pyrenees?
12. During what hours will your Pyr regularly stay alone?
13. Where will your Pyr stay during the night?
14. Where will your Pyr stay when the family is out of town?
15. How long have you lived at your present address?
If less then one year, what was your
previous address?
16. Describe the area where you live (city, suburban, rural, etc.)
17. What type of dwelling is your home (house, duplex, townhouse,
apartment)?
18. Do you own or rent your home? (Your name must be on the mortgage in
order to adopt)
19. If you rent, do you have permission from your landlord to keep a large dog?
Provide your landlord's name and telephone number
for verification.
20. Do you have a securely fenced yard?
21. Describe your fencing and gates (height, chain link, wood, privacy, #
of gates, etc.):
22. For the safety of your Pyr, do you promise to keep your gates locked?
23. During the review process, would you allow a rescue volunteer to visit
your home?

As part of our legal binding adoption agreement, you Pyr must be returned to us
if you no longer want or are able to care for it.
